Evidence of InsurabilityAs a new employee, you are able to elect up to $150,000 for yourself and $50,000 for your spouse with no health questions. If you are a new hire and you are electing an amount that exceeds the guarantee issue of $150,000 employee / $50,000 spouse, health questions apply. Most new and additional elections at Annual Open Enrollment also require health questions / Evidence of Insurability (EOI). Simply complete the form and submit it to Voya for review. You will not be deducted for your requested amount until/unless you are approved.

Evidence of Insurability Form Instructions
Evidence of Insurability Form - Submit this completed form to Voya for review should you elect life insurance that requires Evidence of Insurability.
Portability Form - Submit this form to Voya within 31 days of your employment termination date in order to continue your life benefits. This option allows you to port your coverage to an individual term life policy.
Conversion Form - Submit this form to Voys within 31 days of your employment termination date in order to convert your life insurance to a whole life policy which will be direct billed to your home by Voya.
