2017 Life and AD&D Insurance

In order to provide you and your family with financial protection in the event of your death, Whitfield County Schools offers voluntary term life insurance for employees, your spouse, and your child(ren). The life insurance plan is provided by Voya Financial. The plan includes Accidental Death and Dismemberment coverage, or AD&D. If you die as a result of an accident, the plan also pays either the amount of your life insurance or a percentage for loss of limbs, speech, hearing and more.

Employee $10,000 increments up to a maximum of $300,000, not to exceed 5 times your annual earnings $150,000
Spouse $10,000 increments up to a maximum of $100,000, not to exceed 100% of employee amount $50,000

Evidence of Insurability

As a new employee, you are able to elect up to $150,000 for yourself and $50,000 for your spouse with no health questions.  If you are a new hire and you are electing an amount that exceeds the guarantee issue of $150,000 employee / $50,000 spouse, health questions apply.   Most new and additional elections at Annual Open Enrollment also require health questions / Evidence of Insurability (EOI).  Simply complete the form and submit it to Voya for review. You will not be deducted for your requested amount until/unless you are approved.

Important Note Regarding Beneficiaries and Married Associates Coverage

Please be sure that you have an updated life insurance beneficiary designation.  Please call the Benefits Service Center or access www.totemtools.com to review or update your beneficiary(ies).
 
Married associates may only select Employee Only coverage. To cover eligible dependent child(ren), only one associate may add this coverage.
 

Portability and Conversion

You have the opportunity to continue your life insurance benefit should you terminate employment with Whitfield County Schools.  You may either port your policy to an individual term life plan or convert your policy to a whole life plan upon termination.  Additional information may be obtained by contacting Voya or clicking the links below regarding these two options.

Benefit Reduction

The employee voluntary life benefit reduces to 50% of original benefit on the policy anniversary following your 70th birthday.  The spouse benefit also reduces to 50% on the policy anniversary following spouse’s 70th birthday.

Evidence of Insurability Form Instructions 

Evidence of Insurability Form - Submit this completed form to Voya for review should you elect life insurance that requires Evidence of Insurability.
Portability Form - Submit this form to Voya within 31 days of your employment termination date in order to continue your life benefits.  This option allows you to port your coverage to an individual term life policy.
Conversion Form - Submit this form to Voys within 31 days of your employment termination date in order to convert your life insurance to a whole life policy which will be direct billed to your home by Voya.
